Brain fog can be caused by lots of things. Don't just assume its something to do with your physical health right off the bat - something must have happened after those two years.
Are you depressed? Have you been severely depressed since then?
When someone is "not totally there" its because their mind is always focusing on something else 24/7. Its usually not because of toxin buildup, although it would be prudent to take countermeasures against that as well, considering you live in NA.
But for the most part, its because there's an issue weighing heavily on your mind. You're dragging something with you, when this happens your conscience will be fragmented, subsequently your attention will not be on the present moment. and it seems you've started to become aware of it.
I don't know what's on your mind, or your emotional struggles, but there is a good Taoist exercise which balances the cerebral brain with your feeling brain. The Taoists believe there's a "second brain" in your abdomen, where your true feelings reside. When there's an imbalance of logical reasoning and emotional congruency, often people resort to reason in order to suppress their feelings. This creates a buildup of emotions, and it will cloud your mind.
Search up "Internal exercises by Mantak Chia" and find a .pdf file to download. Look for an abdominal exercise. It will balance out your true feelings with your logical thinking. As I said above, the intertwining of these two can create emotional trauma, which equals brain fog.
