Scattering is a strong possibility or may be an ammonia buildup .

For now , let's get him back to normal, without starting on any other protocol. As whiteshark has mentioned , keep the bowels moving in a simple manner. Get all possible tests done, including MRI's . These will help you narrow down the next approach to take.
Keep posting the updates , so that people here at CZ can help.
