I would guess you may have an infection in that testicle.

Fasting makes the body aggressively look for food in the body. So the bacteria virus etc may go dormant hiding until the fast is over.

Maybe using the Beck Magnetic Pulser. Pulsing the testicle 50 time a few times per week could kill or deactivate the infection.
