Decision: Extract a severely decayed tooth that's severely cracked from decay and is or about to reach the inner tooth nerve pulp chamber where the blood flows. The tooth is still alive but some nerve has died.

Goal: Achieve superior oral health for the long-term and have a biting surface where the extraction site will soon be, eventually.

I'm shooting for an extraction that will be done as holistically as possible. Should the periodontal ligament be removed during the extraction? Should a bone graft from a cadaver be used? Is the Huggins protocol with a socket cavitation where any bad tissue is removed a better way of doing the extraction? The Huggins method seems too sterile and would take more tissue away.

The extraction site would be left empty and the teeth are expected to shift as the body adjusts. Eventually at some point in the future, the empty spot would be replaced to provide a biting surface.

Healing and remineralization has been attempted but it's too far gone. Comments, advice, and tips would be appreciated.
