I use v.hot compresses in conjunction with a cream:

I've had a stubborn case which migrated around my foot. The itch would keep me awake. I found the creams alone were useless. I suppose your own natural defenses play a key role in recovery rate. After about a year getting nowhere, the solutions for me is a combination of keeping my feet ventilated by not wearing tight shoes or boots. Instead loafers so they can be flipped off when they get hot or begin to tingle (convenient while I'm commuting). If I'm at home I apply a very hot compress almost daily or whenever I feel the itch (just for a minute or two). This completely stops the itching for me and I can then apply an off-the-shelf antifungal cream (Daktarin intensive) or pure Tea Tree oil. My skin is then calm for hours and throughout the night! I moisturize on occasion as this hole process does dry the skin and you can be fooled into thinking the dry skin is the rash when it's not. After a month of persistence I'm now on top of it and it's going away. I'd say my skin will be back to normal in 2 more weeks max. You do need to be a bit disciplined about it and pay attention to your body. It's a pretty inexpensive method too.
