Also, if you are having trouble working up to a higher dose, there might be something wrong with the choice of your chelators.

The Cutler protocol for mercury detox prohibits the use of EDTA. However, Cutler says that this only applies to mercury-toxic individuals. (EDTA was the precursor of DMSA. For lead, DMSA is reportedly a lot better.) EDTA, however, would bind to cadmium. I recall reading a post by Cutler in which he seemed to confirm the efficacy of - was it continuous? - IV chelation with EDTA as "the only thing that could prevent a bypass surgery" - but could be only used in individuals without mercury toxicity. I'll try to find that post.
