That is a hotspot there, could be toxins or toxic bile or critters.. irritating things.

Most frequent culprit is pinworms, like A.Vasorum second is a Buski inside the valve itself jamming it open or blocking it or both..

Lemons supply Citric Acid, if you follow Hulda C you will have Citric Acid on hand and can use that instead. It will be more effective..
