Have you tried any sort of chelation (e.g., Cutler method?). You say you've been battling heavy metals, but you don't list any sort of chelation approach.

I and several others on here who have made great progress have done so by focusing on getting mercury and lead out. Judging from your symptoms, I tend to think that the root of your problems is metals and that you won't see any real lasting progress until you start addressing that with DMSA/DMPS/ALA/cilantro/etc.
