I think dermatologists might expect an ulcer (cancerous looking) on bottom lip if you have actinic cheiltis. Maybe they are used to seeing the worst cases in medical textbooks.

I know Accutane can cause sun sensitivity and if one has chapped lips from the the drug drying out skin- the sun can easily penetrate through the lips.
