but if you feel any improvement... like you mentioned you swallow easier then be patient and make sure you use all the cofactors since they are important for speeding up the detox. Salt loading is very important, whenever I feel a little uneasy I do an additional salt loading which take cares of that within half hour. Also other nutrients are important so eat more seafood.

If I were you I would definitely not go with the surgery... instead I will repeat ultrasound in three months or half year to see whether/by how much Iodine supplementation shrinks it.
