Have you mailed the dentists in your areas to get them to help you, or ask them if they could remove the tooth at your house? there must be an understanding dentist out there.

What about Antibiotics , can you tolerate those, and can you get your doc to prescribe some for you? or some anti-fungal stuff?

what about iodine, would that be of any help for you?
