You need to find a new dentist -- at least to handle this aspect of your dental care. Not just because your current dentist doesn't understand why flouride and mercury are so bad for you, but most importantly, because he is't equipped to safely remove Amalgam fillings without causing you more toxicity. Dentists who handle Amalgam removal responsibly have invested thousands of dollars in high force vacuum machines and other items to eliminate as much mercury as possible from the atmosphere during removal. If these extra precautions aren't taken, you can pretty much guarantee that you will get worse following removal because you will inhale and swallow so much.
Look for an IAOMT dentist or a mercury-free dentist in your area. I drove 1.5 hrs to an IAOMT dentist because there was only 1 dentist in my city who claimed to be "mercury-free", but he didn't use most of the safety precautions and then he wondered why most of his patients didn't get better following removal of their fillings (they also didn't chelate). I'm glad I went the extra mile because the other dentist did a very good job.
Anyway -- that's my 2 cents. You definitely don't want to make yourself worse instead of better and I wouldn't waste my time trying to convince your dentist about the dangers of these toxic substances when he's obviously invested in continuing to use them. Life's too short...
