Some of the causes could be:
Lice, Mites, allergy, infected rash, fungus infection such as ringworm, gluten sensitivity.

I had itchy bumps and rough scaley skin on my scalp for years. Only once I did have lice. The rest of the time the bumps were a result of autoimmune gluten sensitivity (Celiac Disease).

You could try treatments for lice and mites.
Use a saturated solution of borax for hair and skin washing. No shampoos or other hair care products. Wash bedding with borax, no detergents.
