Any advice?

I have sinus issues that have gotten worse.They started 3 years back where I had to start using a spray.That worked but it became neccessary that I had a pump all the time now.I have to spray at least once in the middle of the night and about three times a day or my nose gets blocked.I have had blood work done and it turns out im Whey intolerant and have avoided that as much as possible.
I feel sometimes after I spray that my trhoat and ears get itchy now too ,its as if Im allergic and addicted to the spray.
I assume my sinuses is the reason my breathe is now bad too.
My eyes are puffy and I have dark cirlces under them and are itchy.I also have surfers ear ,which is the closing of the ear canals and have tinitus.Could these all be inter related?
any help??
I may get the surgery done for the ears,but probably ot for the sinuses as I have heard that it doesnt really work
