I happen to agree with the great French scientist Antoine Bechamp:  It all comes down to poor cellular terrain, which is where pathogens and disease are most likely to gain a foothold.  What most often creates poor cellular terrain is undernourished, undercleansed and underhydrated cells in combination with toxins.
