Moderators receive a message every time a person hits alert good/bad message. Once one person has pressed the alert button, the message "already alerted" appears under the post text.
When a moderator receives an alert message, the moderator can choose to act or not. If the moderator thinks the message warrants a R rating, or the moderator thinks the message should be hidden or deleted, (in the case of the bad alert), the moderator will act or not, (depending on the moderator).
A moderator can also give a message an R rating, if they think it deserves one, even when the message has not been alerted.
As a moderator of a forum, I'm really glad that messages can only be alerted once, or my inbox would be constantly overflowing :)
