A lot of this sounds like Mercury toxicity to me (I know, because I have it and it can cause all the symptoms you listed). Interestingly enough, my mom also is having the phantom teeth pain (dentist can't find the cause), headaches, heart palps and racing, etc. She has a ton of metal in her mouth and I have been trying for years to get her to remove it. She has truly bad teeth, but not from her own doing. Not sure if you've been tested, but mercury testing is very easy. Also sounds like you should have your adrenals tested. I would suggest a 24 saliva cortisol test. You need to find a doctor or ND who is aware of how to do these tests properly. The tests are very easy to do and results can show a lot. Good luck to you. I too have adrenal fatigue and am on homeopathics to balance them out. I have only been on them a few weeks and started to feel better, but a strong emotional anxiety event happened a few day ago and I started to feel the anxiety come back yesterday. A lot of people have Mercury toxicity and don't realize it. It is sadly very common.
