Repeated antibiotic use; condition made worse with sugar; allergic food responses; digestive chaos, sinus problems; etc - based on the repeated antibiotic use, you'll have systemic fungal candida. This is usually found with the rest of the symptoms that you refer to.

Sinuses are sometimes active to due to stomach acid imbalances, which in your case was helped to a degree by prilosec. Stomach imbalances are also a part of fungal candida issues.
