Hi,
They can do more than just fine you if you don't show up.
If your doctor won't help then you have to stress that you are experiencing anxiety or Depression (look up the symptoms of these disorders and tell him that this is what you experience everytime you are out in public if you must.) If you can get him to diagnose you with something concrete than it should be easier for you to get him to fill out forms etc for you.
