Hi all, wondering if anyone can lend me any insight before I visit my doctor shortly.

I've had urge incontinence since around year 2000 (I am 31 now).

Basically, I often feel the need to go to the toilet and go there but nowhere near a full bladder worth, usually a half bladder worth.

I get the urges worse with alcohol so generally I don't drink often or much, especially beer.

During the day I don't mind drinking water, sometimes I drink plenty, sometimes not much, generally I void a good amount each time.

Sometimes I find it hard to pass urine, it comes out slowly. Especially when drinking and especially when I've had no big urge. Though I remember a couple of times I've been bursting and still coming out slowly.

After every urination, I need to be careful to get rid of "the drops", sometimes I press against the urethra close to where the scrotum meets the body. But I still often get a few drops shortly after leaving the toilet when I need to stand or sit down.

6/7 years ago I saw a urologist (two actually). Had the DRE, urine mass flow rate test (mass flow is normally good so it didn't really tell that much) and inconclusive. Also had an ultrasound, the guy confirmed that I don't completely empty my bladder but otherwise no problems. Because my symptoms are not extreme and can be lived with then this wasn't that big a deal for me.

A year ago I had testicular cancer and one testicle removed. No sign of metastasis and I am on surveillance. I do not think that it is related in any way to my urinary problems but I mention it anyway.

I used to wet the bed when I was younger, even occasionally in adolescence. Even in adulthood, I will wet the bed a handful of times per year. I believe I inherited this from my father.

Sorry for the huge history to my condition! So, about 2 weeks ago I took 400mg ibuprofen 3 times per day as I had a tongue piercing. Since then I have had much more severe urges. Of course I stopped the medication after about 4 or 5 days as I didn't need it anymore anyway. From what I hear the half life is measured in hours but I have no idea of the metabolites.

I still have these bad urges. Basically I will wake in the middle of the night (which I have done on or off over the last few years depending on if I have been drinking liquids in the last few hours). To get to the toilet I must grab a key, put on clothes, find the light etc. etc. No matter how strong my resolve, I usually end up "pulsing" urine out as I am just getting to the toilet.

Previously, this only ever happened if I was severely drunk! Now its happening even if I'm totally sober! It only happens in the middle of the night though (the inability to hold the urine back).

I also smoke marijuana daily and have been doing so for 4 months (and occasionally over the years). I find it difficult to believe there is a connection.

I do find it weird that this escalation is so sudden and I cannot help but think it is linked to the ibuprofen. I found another post on the internet about someone else's experience too:

Otherwise I have also lost much weight over the last 5 months, I was overweight and now I am not (86kg to 68kg).

Any ideas of what may be happening, before I go to the doctor? I have tended to think over the years that this is all in the mind? Maybe I need to do bladder retraining? I definitely do NOT want to take drugs (I already take a PPI for GERD and I don't want more daily drugs in my life). I just find it weird that there is a sudden escalation in the last 2 weeks. I hope it is not diabetes or even prostate cancer (I would be young for that). I also really don't think that it is some kind of urinary infection that orginated from the piercing.

I would really appreciate any insight that anyone can offer? As you can tell, I value the insight here more than I often would from a doctor, good as they are! Any advice for what to ask the doctor for (it will be a totally new doctor as I have moved country).
