yes, all of that; substance abuse and medication can and does cause nutritional deficiencies... and...

Her diet was extreme, HOWEVER if she knew and applied, "as calories fall, nutrition levels must go up" especially the electrolytes (minerals)... she likely would have survived even that - depending on health going in... it is not the calories per se (there is a vague correlation) - it is the high nutrition (mineral) levels in good foods that enable "clean" energy creation.

Refined proteins, sugars, and fats may keep you alive, however they are void of any health sustaining nutrition - and produce a "dirty" form of energy (lots of toxic waste)... and even though you are getting the calories, this type of energy leads to burn out... illness and death.
