Hard to say, in the early days most back-pressure was caused by bile duct blockage. Ascaris, stones or flukes so Mag Malate has always been used.

But the Sky Gods have blessed us with a variety of bioweapons that can trigger this as well.

I would guess that the timing suggests Ascaris suum aka wandering worm...
