If it were during mensturation I would say anemia would be a good possibility.
Being that it is before menstruation it is likely a drop in blood sugar. As progesterone levels drop before menstruation, there is also a drop in blood sugar. This is because progesterone is essential for the production of adrenal steroids, including cortisol. When blood sugar drops cortisol is normally released from the adrenals. This in turn stimualtes the release of stored glycogen from the liver, which in turn brings blood sugar levels back up to normal. Therefore, the drop in progesterone reduces cortisol, preventing the release of glycogen, thus allowing the blood sugar to drop too low.
I recommend eating some nuts or cheese to help stabilize the blood sugar, or try glutamine supplmentation right before menstruating. Glutamine is the only substance other than glucose that the brain can use as fuel without conversion. So it can be used as a substitute for glucose for the brain. It needs to be taken on an empty stomach at least 30 minutes before meals.
