This could be because of B12 difficiency. Have you been tested for it? And what was your value?
Do you know there is a grey area where your value could be too low for you personally or it is not in that area. I think >350 and you probably have enough.
B12 difficiency could be effect of the problems you have.

Have you also looked/tested at lyme disease? Although tests are often false negative (You have to use ELISA and Western Blot, but I think you'll know that).

Are you on a diet Shroom? It helps me with my symptoms. My eye floaters are nearly gone if I'm for a while on a diet. I think I have maybe Lyme Disease btw. I have also chronic fatigue, don't you have?
