You are darn tootin that a Water Fast would probably kill him. I saw a show awhile back that documented a morbidly obese man (probably 500-600 pounds and in his late thirties), and when he went to this facility (not exactly a nursing home but a place you are still in bed 24 hours with people waiting on you),he was still trying to barter with the personel to allow him to eat his snacks. And he wanted and knew he had to change to get better. You can talk to the nursing home about his diet to see if there is any way you can tweak it a bit - it sounds like he shouldn't be eating any sweets, but if he doesn't want to change then there is nothing you can do. All you can do is be completely honest with him about your thoughts on what is happening and how it pains you to see him in this situation. Considering the sacrifices you have made it certainly seems like you are his best bet. This doesn't sound like a situation where you are going to see dramatic changes in a short period of time. Since you said he is drinking some stuff you snuck in plus he has (or has had) some bad infections, you should consider putting some Colloidal Silver in the drinks (or other anti-bacterials, fungals etc.) You should post this on the Ask Tony Issacs forum - he and Luella may have some thoughts on what you can do.
