Increasing infections with immigrants in the States are probably partially the blame here too. I don't think this is the only precursor for the infection rate. Other things have been grossly overlooked; for example, zoonatic infections from pets which is only one way out of many that we can become infected.
I believe if we're ever going to conquer or minimize parasitic infections we need to look at all possibilities with the largest factors involved to the smallest incidents. After evaluating all possible causes education is critical on how these things can be controlled somehow.
After doing some studying on zoonatic infections (as an example, again), it was recognized that by the early 1960's that education was needed for pet owners and that this would become a growing problem. (The actual explosion of home pets began in the early 1900's when the Pekingese dog was brought over from Asia to England.) For some reason, the "word" never got out about parasites and the possibility of infecting homeowners. Just about every home these days has 1-2 pets as family members. I can't help but to wonder if more sophisticated controls could have been employed in terms of better protocol treatments starting with the animals. I've often wondered if this matter was taken more seriously, back then, we might not be in the mess that we're in today with gross infections.
There are, of course, other causes for infections but the above is a good example that needs to be looked at more closely and seriously.
