Your condition is indeed interesting. A simple culture of one of the pustules would reveal the offending pathogen if you wished to find out. There are many other causes of folliculitis beyond bacteria. Maybe look into a condition called Acne Necrotica Miliaris, gram-negative bacteria, demodicosis, scalp rosacea or something in the fungal/yeast spectrum. The fact that you don't respond to Antibiotics may suggest another diagnosis. Or possibly a resistant staph aureus strain (MRSA) which is treatable with a lot of new age Antibiotics preserved for these resistent staph. Sterile folliculitis may well be a real possibility for you and suggests an inflammatory cause rather than a bacterial one. I'm curious as to what your biopsy stated. I'm also curious as to how those with a true bacterial folliculitis are responding to the head shaving. It seems to me that folliculitis decalvans is a primarily inflammatory disorder with lesions being secondarily populated with staph organisms. Those with true staph folliculitis often have issues with shaving as the epidermal damage allows the stpah bacterium to penetrate the skin and cause inflammation. It is for this reason I am very curious as to how they are proceeding....
