Well good luck with the test results. I woudn't worry about the diet right now, the test is done. Really the diet should be normal before testing. So increasing and decreasing choline could work out as normal in the end, maybe. Make sure you are on a normal diet, (not artificially increased choline I mean who would eat more than 6 eggs normally in a day, or decreased choline) for any subsequent tests. You say you keep smelling thimethylaminuria, do you smell of fish? Can you smell yourself? I didn't know that family members are carriers when you are positive. I wouldn't worry about telling your sister she is a carrier she might not be. When I spoke to some experts they said they were studying young children who had genetic TMAU and said when they got to puberty they often grew out of it. I was very surprised at that. It seems it's not all as cut and dried as you think. To answer your question I haven't had a blood test, since the urine came back negative twice it didn't seem necessary.

You say that when you starve yourself is when the odor stops. Have you done any cleansing and colonics etc?
Amonia is a by product of bacteria and you wouldn't have it in large enough amounts to cause death, a lot of people who have bad breath, it is caused by bacteria excreting amonia. This can also happen in the gut. The docs will know if you ask. You said you hope you do have it so people will believe you. Really if you smell bad people will know. You don't need to tell them or explain the odor they usually get it. Although talking about it is a different story. So good luck with the test results and don't worry too much until you have all the results back.
