Sounds like my previous problems with gallbladder, especially since you say he has digestive problems. I had both the chest pressure, something like a belt tightening around my chest, and also the pain between my shoulder blades, especially the right side. I believe I had thrown myself into this by eating a fat free diet for about five years, and then when I started eating fat again, when the cup emptied, the stones got caught in the duct.
My personal theory is that my diet included too many trans fats, and no good fats. See if he connects the pain to french fries or bacon and eggs, or something like that.
Now the sudden sleepiness, to me, is worrisome, but I would not be surprised is he has become diabetic. Let the VA do all the tests. I agree with HarleyGirl that you need a good diagnosis. When I was diagnosed diabetic, I had to sleep all the time. Of course, all that Sugar circulating in the blood makes one tired. Takes all the energy of the body just to handle that.
The normal BP is an excellent sign!
He seems to be at a crossroads here. The VA will undoubtedly order tests and then prescribe medication. He can go that route. Or he can radically change his diet (and you should, as well. I don't know any of us who wouldn't gasp at pizza and Coke) and try to get off those meds as soon as possible. Diabetes is a lifestyle disease. IMO, so is gallbladder disease. I ate out a lot and did a lot of take out. Please read this link where I realized where I was after being off meds for diabetes, hypertension and Depression for three weeks. Since that post, I had to take two diabetic pills on two different days, due to not controlling my food, I am sure.

In order to stay off those drugs, I have to monitor my diet. But that's OK. I eat nothing from a box, only fish and tomatoes from a can, nothing frozen except what I prepare fresh and freeze myself, and no fast food. The diabetic diet of one protein, one starch, one vegetable, milk and bread did not help me. I had to find my own diet of protein and vegetables.
There is life after disease! There is healing after disease! Please don't shoot the messenger because we are talking bad about pizza and Coke. We have all done pizza and Coke, in our past. But we wanted to be well more than we wanted the pizza and Coke. Your brother can too.
By the way, there are no coincidences. The fact that your brother is having these problems may just be your own wake up call to change your diet for the better. God bless you and your family and may you all have fantastic health!.
