Dr. Javier Morales Experiences Survey WEBMASTER

I was looking at the results of this survey as I was thinking of having my teeth worked on, and although many of the folks who took it were not satisfied, there is no place on the survey that notes WHY - other than the list of symptoms which is not truly indicative of the work done, just whether or not the persons' symptoms abated.  Since mercury needs to be removed, not just the teeth, this is not a good snapshot of WHY people were not pleased - are they regretting it for reasons other than what you have listed?  Can you add questions that refer to the work done, were they happy with the procedures, the finished product, etc?  What would they change, what were they dissatisfied with, and also a place for people to add things if they need to?
