OT: Emergency card info

Somewhat off-topic, but I felt it would be best here and/or other "full" cleansing forums.

I religiously keep an emergency card in my purse, just in case. Before the MC, I would like to know what information, in addition to my current info, would be recommended to add. Here's what I have as a standard:

Name
DOB (date of birth)
Home phone
Cell Phone
Emergency contact (EC)
EC phone 1
EC phone 2
Med Info *
Current Info **

My med info contains any Rx meds and regularly taken OTC items including headache meds, supplements, etc (anything I take daily or semi-daily). I also state I'm a smoker, PPD (packs per day) and years.

For current info I have [1] weaning off SSRI (who knows, this may be important in emergency health care) and [2] "Likely on a cleanse: parasite, liver, kidney, Master Cleanse" (in hopes they've ever heard of these).

IYO, is there any other info I should add especially pertaining to a Master-Cleanse or other? Any info you feel is "TMI"? (too much info). Tweet
