Is this formula good for osteoporosis also?

Yes, any condition where there is a loss of collagen and elastin. These include osteoporosis, osteoarthritis, emphysema, diverticulitis, etc.  The difference between osteoporosis and osteomalacia is the loss of collagen in the osteoporosis.  The collagen matrix of bone is where the silica is that allows the mineralization of bone.  It is the collagen matrix that gives bone the majority of its strength, no the minerals.

What would be the dosage for a very small toy poodle?

Recommended dose is 1/4 teaspoon 3 times daily.
