If the infection is outside the eardrum then the most common reason is water in the ear.  A really big mistake people make is using cotton swabs in their ears after bathing or swimming.  Most of the time they just push the wax over the water trapping the water in the ear.  The warm moist environment is a perfect environment for pathogen growth.  This is why I don't like people putting anything with water in their ears including hydrogen peroxide or colloidal silver.

Infections can also happen behind the eardrum.  This can be from trauma, such as a ruptured eardrum, or from other sources such as an abscessed tooth.

If you can get access to ozone this is the most effective way to deal with ear infections.  Lay your head sideways and blow the ozone in to the ear under very low pressure.  The ozone weighs more than air and will sink in to the ear canal and go right through the eardrum killing any pathogens as it goes.
