Rhinoplasty and insurance

I have always wanted to have the "hump" on my nose fixed, but have been putting it off becuase of cost. Within the last 2 years though, my allergies have been very bad causing several chronic sinus infections. My doctor also noticed that I have a "slightly" deviated septum, but more of a problem has been the polyps in my nose have become very swollen and at times have made it very difficult to breath. I believe that my insurance will cover the surgery to fix these problems, but how much ( or what percent) can I expect to pay out of pocket to have the hump removed?
