Your TSH says that you are likely hypothyroid. Do you happen to have labs on Free T4 and/or Free T3?
Hypothyroidism can cause the hair loss, and then if your ferritin is too low you can have problems with it growing back...this was my problem. A good ferritin level to shoot for is 70- 90.
Try taking your temperature in the morning before you get out of bed, under the arm with a glass thermometer. Your temp should be between 97.8- 98.2 consistently if your thyroid is working fine. Try taking it for a few days and see what you find, this will tell you how much of a thyroid problem you have. The labs would be good to get too.
